Boarding and Daycare Contract
For the health and well-being of all dogs in our facility, we ask that you please ensure your dog is free of fleas and ticks as well as being reasonably clean and groomed before their visit. If we find any such pests or detect and unbearable odor, we will give flea/ tick treatment and/ or a bath at owner's expense.
For boarding, the owner should provide food either individually packed per meal or in an airtight container with feeding instructions for longer stays. DDOS of has dry kibble available for $3.00 per meal if you so chose.
It is common for dogs new to boarding, to eat very little during their first few days. Check this box if you would like DDOS to incentivize feeding by adding canned food to their normal food.
If your dog(s) are not picked up or future arrangments made within THREE (3) days of your scheduled pick up date, Animal Control will be notified and your dog will be taken due to abandoment. (If your plans change for whatever reason we can be reached via Phone call, Facebook Messenger, or E-Mail to update us of any changes needing to be made to your booking.)
Any daycare dog not picked up before 6 pm will be charged a late fee per additional 30 minutes. At 7 pm the dog will be placed in overnight boarding.
Boarding stays in excess of ONE (1) week, may be asked to pay 1/2 of the invoice upfront.
I understand and agree that in admitting my dog(s) to Dog Days of Sommer (DDOS) the owners and their employees relied up on my honest representation that my dog(s) has / have not harmed or shown any type of agression or threatening behavior to any other dog or person and is in good health.
I further understand and agree that I am solely responsible for any harm caused by my dog(s) while they are attending DDOS. I agree to indemnify, defend, and hold harmless DDOS and its employees, officers, directors, and volunteers from all claims, damages, or causes of action arising from my dog(s) attendance at doggy daycare or boarding. I understand that it is my responsibility to pay for all dmages arising from my dog(s) attendance at DDOS.
I further understand and agree that DDOS, its staff, and volunteers will not be held liable for any problems that may develop (i.e., injuries, fleas, ticks, illness, ect.) provided reasonable precautions were taken and care were followed. I hereby release them from any liability of any kind whatsoever arising from my dog(s) attendance at DDOS.
I further understand and agree to waive and release DDOS, its employees, officers, directors, volunteers, and agents of any kind from liability of any nature for any injury, death, or loss of my dog(s) resulting from DDOS actions or the actions of my dog(s) or any other dog while in the custody of or on the grounds or surrounding area of DDOS.
In the event that my dog(s) cause injury to another dog or person while at DDOS, I agree to indemnify and subrogate DDOS from any action which may be brought against it and for any defense, settlement, or judgement entered against it. I will assume all liability for the actions of my dog(s) and agree to maintain personal liability to cover me in the event of such an incident.
I further understand and agree that any problem arising with my dog(s) will be treated as deemed necessary by the staff at DDOS. I assume full financial responsibility for any and all expenses involved, including but not limited to veterinary, property, and medical bills.
In the event of an emergency rendering the DDOS facility unusable or other such reason as deemed necessary, I give my permission for the owner or employee of DDOS to take my dog to their residence.
I certify that I have read, understand, and agree to the rules and regulations set forth by this document. I further agree to abide by these rules and regulations and accept all the terms, statements, and conditions of this agreement.
