I, the undersigned, warrant that my dog is current with the vaccinations required by law and that my dog is healthy to the best of my knowledge. If my pet is hurt or becomes ill, Dog Days of Sommer (DDOS) has permission to take them to a veterinarian of our choice. Should the pet's preferred veterinarian be inaccessible, an emergency vet may be utilized. Expenses will be paid promptly by the pet owner unless injury is the direct result of negligence by DDOS staff. We will not be held responsible for any sickness or injury caused by the pet itself during grooming. DDOS is not responsible for any accidental death of pet of the nature of pre-existing conditions or natural disasters (fire, storm, flood, etc.) DDOS will not be held responsible for the stressful effects grooming may have upon an elderly pet.
DDOS reserves the right to charge additional fees for services we consider over and above the norm covered by our standard rates.
DDOS also reserves the right to refuse service to customers whose pet may pose a threat to our staff or other pets in our care. Pet owner agrees to be responsible for any property damage caused by their pet.
Cancellation and No-Show Policy: Cancellations and rescheduling must be done no less than 24 hours before appointment time. Not adhering to this policy will subject the account to a mandatory $40 deposit requirement to schedule future appointments.
I, the undersigned, agree that I have read and understand the rules and conditions for grooming services provided by DDOS. I understand that my dog, my guests, or I may be recorded or photographed and that these images become the property of DDOS. I agree to release, hold harmless, acquit, will indemnify, and forever discharge DDOS from all and in all manner of action and actions, suits, damages, judgements, executions, claims and demands whatsoever, in law or in equity, which I or any other person I have with me during grooming at DDOS.
